Stephens Farm is a wonderful resource for individuals that have Traumatic Brain Injuries (TBI) in Northern Colorado and surrounding communities! Opening in 2025, 2730 @Adeo is undergoing renovation to offer more individual living options for people with brain injuries.

Eligibility

Anyone over the age of 16 who has a brain injury may apply for our programs. Almost all applicants for our services are eligible for Medicaid funded long term care. To apply for Medicaid, you may apply here.

You must also be eligible for the Brain Injury Waiver if enrolled in Health First Colorado (Medicaid). (Note: you do not need to be on the waiver; you simply need to ensure you are eligible.)

Eligibility Criteria
  • Diagnosis of brain injury
  • 16 years or older
  • Brain injury occurred prior to 65th birthday
  • Require long-term support services at a level typically provided in a nursing home
  • Meet financial, medical and program criteria to access services under a waiver
